It has always surprised me that DW canon events can be so much darker than Torchwood, but they get away with it because of how it shown visually, ie without any blood or swearing.
The fate of the Family of Blood was, I found, particularly cruel - that death wasn't great enough a punishment for them (they would have died in short order without their engery source) but the Doctor found it neccessary to punish them in such a way as they wouldn't die and could never escape unless he released. The fact that he went back once a year to check that one of the was still trapped pretty darn creepy. It made me think that if the Doctor ever went bad he'd go really bad indeed.
